Vacuna rusa: qué dice un informe que no publica Rusia y habla de efectos adversos
Se filtró un documento de la Anmat donde se describen 12 efectos adversos considerados ‘graves’ de la vacuna rusa.
Cuando se ofrece poca información sobre un tema que inquieta mucho, es común que en las redes sociales se reponga, mal o bien, el bache, sea con información falsa (fake news) o con datos verdaderos que algún actor -impulsado por intereses personales- hace rodar. Esto último quizás explique por que este lunes circula en Twitter un informe interno de la ANMAT fechado el 23 de diciembre, precisamente (confirmó Clarín, tras chequear la autenticidad del documento) uno de los que sirvió de apoyo para que ese organismo “recomendara” al Ministerio de Salud aprobar el uso de la vacuna rusa (Sputnik V) contra el coronavirus, en personas de hasta 60 años.
Se trata de un texto firmado por Nélida Agustina Bisio, técnica de la Dirección de Evaluación y Registro de Medicamentos (DERM). En cinco páginas, la técnica informa a su superior Marcelo Alberto Carignani, director del Instituto Nacional de Medicamentos (INAME), dependiente de la ANMAT, los datos intermedios de fase 3 (recogidos hasta mediados de diciembre) que el Centro Gamaleya presentó al Ministerio de Salud de la Federación Rusa.
Se deduce que, como no es un documento menor, Bisio puso en copia a dos jefes del propio Carignani: Manuel Limeres y Valeria Teresa Garay, nada menos que quienes ocupan el segundo y tercer puesto en la línea de mando de la ANMAT, encabezada, desde ya, por el ministro de Salud de la Nación, Ginés González García.
El estudio en cuestión es un corte (o endpoint) que ya había sido adelantado por Clarín: son 12.296 voluntarios de 18 a 87 años, parte de los 40.000 que incluye la fase 3 de la vacuna Sputnik V. Como se sabe, más allá de que la vacunación en Argentina comenzará este martes, los datos volcados en este documento (hasta ahora, confidencial) no fueron todavía publicados en ninguna revista científica, algo que ocurriría en enero.
¿Qué dice el informe? Al menos tres cosas importantes. La primera, que la efectividad de la vacuna es superior al 91,4% deducido de la fase 2, y que llegaría al 96%.
La segunda, que los datos provistos en relación a los mayores de 60 años son “flojos”. Por eso los técnicos a cargo del informe explican que “en base a estos datos para mayores de 60 años, se recomienda esperar el reporte de más datos de eficacia, inmunogenicidad y seguridad”.
La tercera, que hubo 12 eventos adversos considerados “serios”, de los cuales 3 ocurrieron en mayores de 60 años que habían recibido la vacuna. El texto detalla que se trató de un caso de cólico renal, otro de trombosis venosa profunda y un “absceso en miembro”.
Por otro lado, el “Análisis de seguridad” reporta 8.704 eventos adversos leves en 4.401 voluntarios. “El evento adverso más frecuente observado fue síndrome similar a gripe en las personas que recibieron la vacuna en comparación al placebo”, dice el informe, y agrega que “se registraron reacciones locales en el sitio de inyección”, más frecuentemente en el grupo vacunado que en el placebo.
Letra chica
Un ensayo de doble ciego supone que la mitad de los voluntarios reciba el medicamento que se intenta testear, y la otra mitad, placebo (o sea, nada), sin tener idea -ni los participantes ni los responsables del estudio- quién recibió qué. Antes de comenzar el ensayo se define en qué puntos (o endpoints) se abrirán datos parciales, para hacer un seguimiento minucioso de la investigación.
En este caso se había determinado que con “al menos 20” contagios de Covid-19 se revelaría una porción de los datos. Es por eso que el informe explica que “fueron verificados, al momento del cierre de la base de datos y se han incluido en el análisis primario de eficacia, 12.296 sujetos y 27 casos de Covid-19 de acuerdo a lo planificado para el primer punto de corte de análisis interino (al menos 20 casos)”.
Según lo evaluado, “la población incluida en las dos ramas del estudio no demostró diferencias estadísticamente significativas en relación con las variables de edad, sexo, peso o raza”, y deducen “un perfil de seguridad aceptable” de la vacuna rusa.
Un dato importante (pero del cual no se dan muchos detalles) que es varios de los participantes tenían comorbilidades: casi el 26% de los voluntarios del grupo “vacuna”, y 27,3% del grupo “placebo”.
En cuanto a los mayores de 60 años, aunque Rusia anunció este fin de semana que finalmente aprobó el uso de emergencia de la Sputnik V para esa franja etaria, la autorización que dio el Ministerio de Salud de Argentina no incluye, por ahora, a esa población. Y, de hecho, el informe interno de la ANMAT lo justifica.
La argumentación es concesiva, sí, pero también cautelosa.
Primero explican que “no se observa (en cuanto a la eficacia, comparada con los menores de 60 años) una diferencia estadísticamente significativa en los mayores de 60 años, lo que podría deberse a la baja incidencia de casos registrados al momento del corte”.
Luego, agregan: “El perfil de seguridad en mayores de 60 años no difirió del perfil de seguridad observado en el resto de la población analizada. Considerando que los sujetos mayores de 60 años expuestos al producto en investigación fueron 1.029, se sugiere un nuevo análisis de seguridad a la espera de eventos adversos que podrían observarse en mayor número de sujetos expuestos”.
Y un tercer párrafo aclara que “el reporte analizado no arroja datos en relación a la inmunogenicidad en mayores de 60 años”, de lo que se desprende que “en base a estos datos para mayores de 60 años, se recomienda esperar el reporte de más datos de eficacia, inmunogenicidad y seguridad”.
